Unable to twist open bottle caps, getting winded after climbing just two flights of stairs, needing support to stand up from the sofa… Many people dismiss these signs as normal signs of ageing.
However, in 2025, the Asian Working Group for Sarcopenia: Expert Consensus, published in a sub-journal of the authoritative international journal Nature, moved the critical intervention window for sarcopenia prevention and management forward from the traditional age of 65 to 50 for the first time.
This means age 50 marks the watershed for muscle health.
01 Your muscle mass begins to decline rapidly starting at age 50



Few people know that human muscle mass peaks at around age 25. After 30, muscle mass declines by 3% to 8% every decade. Once people turn 50, muscles enter an accelerated loss phase — declining by more than 1% annually. Leg muscle mass drops by 1%–2% each year, while muscle strength decreases by 1.5%–5% per year.
The harsh reality:
By age 60, up to 30% of muscle mass may be lost; for those over 80, nearly half of muscle mass disappears.
This is not “normal ageing”. It is sarcopenia, a disorder characterised by reduced muscle mass, declining muscle strength and impaired physical function.
How severe is the situation in China? Statistics show the prevalence among community-dwelling older men is 12.9%, and 11.2% among women. Up to 65.9% of seniors in elderly care institutions face sarcopenia risks. The prevalence reaches roughly 20% for people over 60, surging to 67.1% among those aged above 80.
Sarcopenia is far more serious than simply “losing weight with age”. It drastically raises fall risks, triples the risk of osteoporosis, triggers a sharp deterioration in capacity for daily activities, and is directly linked to higher mortality risk.
However, there is good news: ages 50 to 64 constitute the golden intervention window. People in this age group generally enjoy a sound physical foundation with stronger muscle regeneration capacity, and they respond far more sensitively to nutrition and exercise than older adults.
It is never too late to start preserving your muscle mass.
02 Two-Minute Self-Check: Is Your Muscle Mass Slipping Away?
No hospital visit or expense required. Three methods enable rapid preliminary screening.
Method 1: Finger-Ring Test
Form a circle with the thumb and index finger of both hands, and wrap it around the thickest part of your calf.
If the finger ring perfectly fits the calf, or there remains space inside the ring after encircling, the test yields a positive result — indicating elevated sarcopenia risk. Further medical assessment is recommended.
Method 2: Calf Circumference Measurement
Locate the thickest section of your calf (usually slightly above the midpoint). Ensure the soft measuring tape runs parallel to the ground, wrapped fully around the calf, lightly touching the skin without squeezing soft tissue.
Male: < 34 cm
Female: < 33 cm
Values below these thresholds signal higher sarcopenia risk.
Method 3: SARC-F Questionnaire
Answer the following five questions:

A total score ≥ 4 points indicates high sarcopenia risk.
Important Notes:
The SARC-F questionnaire boasts high specificity (86%–91%) and effectively identifies high-risk populations, yet its sensitivity is relatively low (27%–39%). Used alone, it may lead to missed diagnoses. Therefore, combining the finger-ring test, calf circumference measurement and SARC-F questionnaire is advised to improve the comprehensiveness and accuracy of screening.
If self-test results are concerning (SARC-F ≥ 4 points, calf circumference below cut-off values, or positive finger-ring test), visit a hospital promptly for professional muscle strength assessment (e.g. handgrip test) and muscle mass evaluation (via DXA or BIA testing) to confirm diagnosis.
03 The 3-Step Plan to Preserve Muscle Mass

Step 1: Get the Right “Building Materials” — Protein
Muscles can be likened to a house, and protein is the bricks and tiles. Without bricks and tiles, the house can never be built.
Core target: Adults aged 50–64 should consume no less than 1.6 g of protein per kilogram of body weight every day.
Example: A middle-aged adult weighing 60 kilograms needs more than 96 grams of protein daily.
How to eat properly?
Distribute protein evenly across three meals. Do not consume most of your protein at dinner.
Aim for 25–30 grams of high-quality protein per meal (approximately: 1 egg + 1 cup of milk + 50 grams of lean meat).
Recommended sources: fish, shrimp, lean meat, eggs, milk and soy products.
For those with vitamin D deficiency: Supplement 800–1000 IU of vitamin D daily and get adequate sunlight exposure.
Important reminder: Steer clear of misconceptions such as “a vegetarian diet equals good health” and “eating less meat prevents hypertension, hyperglycemia and hyperlipidemia”. For people over 50, blind vegetarianism is equivalent to tearing down your house.


Step 2: Mobilise the “Construction Crew” — Exercise
Bricks and tiles alone are not enough; you also need workers to carry out construction. Exercise acts as the “construction crew” that activates muscle synthesis.
Resistance training is the core
(2–3 times per week):
Dumbbell lifts (or use water bottles filled with water as alternatives)
Resistance band workouts
Wall sits
Seated leg raises
The appropriate intensity means your muscles feel slightly sore after training, and soreness fades by the next day.
Aerobic exercise as supplementary training
(3–5 times per week, over 30 minutes per session):
Brisk walking, jogging, swimming, stationary cycling
Accumulate a minimum of 150 minutes of moderate-intensity aerobic exercise every week.
Balance training to prevent falls
(3–5 times per week, 5–20 minutes per session):
Single-leg standing, tandem standing, Tai Chi, Baduanjin
Keep this principle in mind:
Multimodal exercise (resistance training + aerobic exercise + balance training) delivers the best results, while single-type exercise only brings limited benefits.

Step 3: Technology-Assisted Intervention – Whole-Body Vibration Therapy: Preserve Muscle Mass While Resting
For older adults with poor physical stamina, joint disorders, or those unable to perform high-intensity training, conventional exercise often leaves them willing yet unable to keep up.
The Guidelines for Rehabilitation Intervention in Geriatric Sarcopenia (2025 Edition) specifically recommends an alternative intervention: Whole-Body Vibration Therapy (WBV).

Step 4: Steer Clear of “Muscle Thieves”
Certain daily habits are silently draining your muscle mass:
🚫 Smoking → Increases sarcopenia risk by 2.09 times
🚫 Sedentary lifestyle → Less than 150 minutes of aerobic exercise weekly and fewer than 2 resistance training sessions mean muscles lack adequate mechanical stimulation
🚫 Uninformed weight loss → Unintentional weight loss ≥ 2 kg within 6 months serves as a warning sign of sarcopenia
🚫 Extremely low-protein diets → Severe shortage of raw materials for muscle synthesis
Sarcopenia is not incurable. The key lies in early detection and timely intervention.
Age 50 is not the beginning of ageing — it is your final window to preserve muscle mass.
